This form is for purchasing duplicate awards on or after the 21st Annual G.A.N.G. Awards. If any awardee name(s) cannot be automatically verified with this order form, or you need to order an award for a year prior to the 21st Annual G.A.N.G. Awards, please use the order request form.

It is highly recommended to use this order form when possible so that we may process your order in a timely manner.
